
About Health Essentials Health Essentials (“HESS”) has assembled a comprehensive integrated package of healthcare services focused on managing the costs associated with post-acute services through end-of-life hospice. These services include subacute SNF services, hospitalist/intensivists, high risk case management, professional and traditional home health services, board and care facilities, DME and pharmacy services, hospice services, improvement to RAF scoring, effective management of admission and readmission of identified high risk patients.
Health Essentials principal clientele include hospitals and hospital systems, foundations, integrated delivery systems, managed care organizations, health plans, and medical groups/IPAs. The company’s focus is the post acute care band of the inpatient healthcare continuum. We place emphasis on 30 day readmission rates when approaching hospital and health plan executives about making HealthEssentials an extension of their strategies for Accountable Care Organizations (ACO) or Patient-Centered Medical Homes (PCMH) as they strategically prepare themselves for initiatives related to health care reform.
The conditions driving the high readmission rates reported in the New England Journal of Medicine are the usual suspects: CHF, COPD and Pneumonia, as well medical complications following a surgical procedure, often Pneumonia. These patterns are particularly troubling to hospital executives who are trying to turn their battleships around in time to be ready for health care reform in all of its facets. Bundled payments will be phased in over the next few years and become permanent in 2015. Hospital readmission rates will become public in 2012, and penalties of 1-3% of all revenue from Medicare DRGs payment will be levied beginning in 2013 starting with excessive readmission rates for Pneumonia, CHF and Acute Myocardial Infarction (AMI).
Kim Phan
Ms. Phan is co-founder of Health Essentials LLC., a medical services organization (MSO) providing management services for medical groups, home health agencies, hospices and other ancillary companies. She is also co-founder of hospices, a home health agency, DME and closed door pharmacy. She received her BA in Economics and MBA from University of California, Irvine. She was General Manager for Caremore Touch, an I-SNP. She was Regional Vice President of Cove Healthcare, a venture funded Hospitalist company. In addition, she has served as Associate Administrator for Orange Coast Memorial Medical Center, Associate Administrator at FHP Hospital and Administrator of FHP’s 99-bed skilled nursing facility.
Phil Scott
Mr. Scott is President of Health Essentials. He has an MBA (Summa Cum Laude), from the University of San Diego ad a BS Degree in Chemical Engineering from California Institute of Technology. He is also a Chartered Financial Analyst designee. Phil has twenty years of financial, valuation, corporate advisory, merger and acquisition and restructuring experience He currently owns a valuation company and prior to joining HealthEssentials was VP of Development for Heritage and HealthCare Partners. He was also co-founder of the Camden Group, a healthcare consulting practice acquired by HealthCare Partners.
Christine M. Mlot
Christine M. Mlot MD, CMD is co-founder and President of GeriNet Medical Associates, a specialty physician network which provides physician and Nurse Practitioner services to skilled nursing and acute care facilities in California and Nevada. She has 16 years experience working in California nursing facilities. She is a Certified Medical Director through the American Medical Directors Association; and has a certificate in hospice and palliative medicine through the American Academy of Hospice and Palliative Medicine. She is the Medical Director for Hospice Touch, Chief Medical Officer of the Veterans Home of California-Barstow, Medical Director of San Diego’s Program of all-inclusive Care for the Elderly (PACE) program, and serves as Medical Director for several community skilled nursing facilities in San Diego County. Additionally, Dr. Mlot is President of ComfortRx, a closed door pharmacy.
