Hospitalist/Geriatric specialists
HealthEssentials utilizes GeriNet physicians and nurse practitioners who specialize in geriatrics. These providers have special training and experience to help them deal with the complex needs of residents. GeriNet will provide physician and nurse practitioner services to residents.
Frequent visits by medical providers
Through frequent visits by a GeriNet physician and care management by nurse practitioners, the HealthEssentials from GeriNet program provides residents with the increased medical attention they need to maintain their health status and quality of life, while helping to reduce the incidence of serious acute episodes that often result in hospitalization.
More care provided on-site
As much needed care as possible is provided on-site, at the facility. This is much less disruptive and more cost-effective than continually transporting the resident to other locations for needed services. This includes more treatment for acute episodes in the facility rather than at the hospital.
Improved communication
The program improves communication between the physician, nurse practitioner, facility staff, resident, and family members. Good communication is very important to making sure that the best care decisions are made in a timely manner. With a comprehensive medical record that can be accessed by all the care providers in the HESS delivery system the level of care is greatly enhanced fully documented.
Improved continuity of care during acute episodes
The full spectrum of care required in the HESS delivery model is a “team” approach. Through a coordinated effort of the entire team, which is fully documented and available to all the team members, the best possible care and outcomes at the lowest possible cost is provided.
