Our Services The primary services that HESS provides to execute on its mission are:
Post acute inpatient physician services at SNF, assisted living facilities, board and care, and in the home.
High Risk Case Management Program including placement of sub-acute patients in SNF and providing Nurse Practitioners (NPs) to provide care for home bound patients. This program is also provided to post acute custodial, assisted living and hospice patients.
Increase RAF or HCC by proficient coding initiatives to improve global medical funding for HESS clients.
Home health and rehabilitation services.
Effective management of DME and Pharmacy services.
Referral to hospice when appropriate.
Integrated information and communication systems to improve coordination of care.
Hospitalists/ Intensivists where required.
